Bright Eyes Midwifery and Wild Rivers Women's Health is Closed


After 8 years of providing care to the community, I have made the decision to close Bright Eyes Midwifery and Wild Rivers Women’s Health LLC, in Gold Beach OR effective 11-21-2022.

It has been an honor to care for you over the last several years. With this change, I will be moving to Coast Community Health Center (CCHC) to provide women's healthcare (well-woman exams, contraceptive care and other women's health needs) at their Port Orford Health Center, and newly opened Brookings Community Health location, starting in January 2023. Coast Community Health Center has been serving Coos and Curry Counties for 12 years. CCHC’s medical team of tenured doctors, nurse practitioners, and behavioral health professionals is led by Dr. Ann Kellogg, DO, and I will be working with this seasoned team of medical professionals to continue to provide care to any of my patients who wish to transfer to Coast Community Health Center.

I would be honored to continue to serve your women’s health needs at Coast Community Health Center at either location, as well as having my new colleagues at CCHC provide you with any primary care services you may need. With this change, I am saddened to say I will not be providing obstetric services at CCHC.

Transferring your women’s healthcare services to CCHC is easy. You will need to make sure you fill out a new patient packet, make an appointment, and complete a Release of Information (ROI) https://www.coastcommunityhealth.org/appointments/ so that your records can be moved from Bright Eyes Midwifery & Wild Rivers Women’s Health LLC to Coast Community Health Center. If you desire to move your women’s care to another provider, you will need to choose a new provider to make sure your medical care continues (see reverse side of this letter).

CCHC accepts all insurance carriers. As a non-profit health center, CCHC also offers a sliding fee for out-of-pocket services for those who qualify and offers the no/low cost Contraceptive Care Program (CCARE) to patients. Joining forces with CCHC allows me to do what I enjoy most, taking care of my patients, while bringing expanded programs to all patients.

Your Medical Records

Effective 11/22/2022 we have retained Cariend, a medical records custodian, to manage the records for the next 21 years. Cariend is not a medical provider and does not handle billing, medications, or referrals for the closed practice.

Requestors should be prepared to complete the Records Release Authorization Form and provide a copy of government-issued photo ID, as required by the HIPAA laws. The fastest way to process requests for copies of medical records is to visit Cariend’s website and submit your contact information at the link below – you will then be sent an email with instructions to securely upload your authorization form and ID: https://www.cariend.com/request-records/

If you do not have access to a computer or internet, Cariend can be reached by phone at 404- 991-7080 – please note the HIPAA laws do not allow medical records to be released based on a phone or email request.

Do you wish to find a new doctor to provide care to you outside of CCHC?

For help finding a new doctor, call:

Your health Insurance Company, or; Call Dr Laudert or Dr Battey at Curry Health Network at 541-247-3000 to be seen for your obstetric and women’s health needs.  Make sure your new doctor gets a copy of your medical record. They can be requested by your new provider, or yourself through Cariend, as mentioned above.  If you need to transfer your records prior to 11/22/2022, when Cariend’s service begins, please call my office at 541-425-5311 before 11/21/22 or have your new or current provider send a Records Release Form to fax number 541-425-5577.


Please allow Cariend up to 30 days to release your records during this transition.

Even if you get your care elsewhere going forward, I will continue to remain part of our community.
